Getting ready to enroll in Medicare?

You have a total of seven months to sign up for Medicare – three months prior to your 65th birthday, during the month of your birthday, and for three months immediately after it.

But no matter when you enroll in Medicare, you must prove your age and your eligibility. So, you will want to make sure you gather all the documents and required information you’ll need before you start the process. Do You Have the Necessary Documents for Enrolling in Medicare?

You need to provide the following documents . . .So, you can prove . . .Other information you should know:
Your Birth Certificate  
Your Social Security Card
You are who you say you are    Other acceptable documents to prove birth may include medical, insurance, and immunization records.  
Naturalization Certificate or   Permanent Resident Card
(Green card)
You are a U.S. citizen
or legal resident
Documents may include a passport or another form of legal documentation to prove citizenship or legal residency. If you’re not a U.S. citizen, you can show another form of residency such as a green card.
W-2 Forms Your work historyThese forms are required if you worked in the two years prior to enrolling in Medicare.
A statement from the Civil Service Retirement System (CSRS)
or
Federal Employees Retirement System (FERS)
You receive a federal pension 
Your Statement from ssa.govYour Social Security work credits 
Discharge Papers Your U.S. military serviceProvide this document if you served in the U.S. military before 1968.
Name(s) and Social Security Numbers for:
-all spouses, past or present;
-children under 18 years old
Immediate members of your family

Name(s) and Social Security Number(s) are also required for your disabled children, up to 22 years old

Already receive social security or disability benefits?

There’s no need to provide any other documents when you sign up for Medicare.
